All schools need to complete the membership data card below and submit dues of $60 to the address listed on the invoice.  You will need to fill out the Data Card and Invoice for your school. The data card may have a digital signature and is a fillable PDF. There is a new name and address on your invoice as of 2022.  Please alert your bookkeeper to the change.  Checks going to the old address will hopefully be returned to you but may be lost forever.  

Membership in the Milwaukee CFL entitles a school to enter students into qualifying tournaments for the NCFL Grand National Tournament.  If you school requires a W9 in order to process payment, it can be found below the membership forms.
